Privacy Policy
This page explains what we collect when you enroll in the PAMSA member health benefit, how that information is used, and what stays private from PAMSA.

What we collect
- Enrollment details you provide: your name, phone number, email address, health plan, the program you are interested in, and any notes you add.
- Clinical information gathered during your visits, such as intake answers, measurements, lab results, and medications prescribed by Mino MD physicians.
- Basic appointment and scheduling records, including visits booked, completed, or cancelled.
How we use it
- To confirm you are eligible for the PAMSA member benefit and to enroll you in the program you selected.
- To have a member of the Mino MD care team reach out, schedule your first visit, and deliver ongoing care.
- To bill your existing health plan for covered services, so your cost stays at $0.
What PAMSA sees
- PAMSA does not receive your individual health records, diagnoses, medications, or visit notes.
- PAMSA receives aggregate, group-level participation data only — for example how many members enrolled — never information that identifies your personal care.
- Participation is voluntary. Enrolling or declining has no effect on your membership or employment.
Who else may receive information
- Licensed Mino MD clinicians and care coordinators involved in your treatment.
- Your health plan and its administrators, for eligibility verification and claims.
- Service providers that help us operate the program — such as scheduling, secure messaging, and records systems — under written confidentiality obligations.
- Authorities when the law requires disclosure. We do not sell your information or use it for advertising.
How we protect it
- Your health information is handled under HIPAA and applicable state privacy laws.
- Records are stored in access-controlled, encrypted systems, and access is limited to staff who need it for your care.
- We keep records for as long as required by medical record retention laws, then dispose of them securely.
Your choices
- You can ask for a copy of your records, request a correction, or ask us to stop contacting you.
- You can withdraw from the program at any time.
