These terms explain how the PAMSA member health benefit works, who can use it, and what you and the Mino MD care team can expect from each other.

About this benefit
- The PAMSA member health benefit gives eligible members access to physician-led weight-loss, metabolic, and peptide care delivered by Mino MD.
- Enrollment is voluntary. Choosing to enroll or not enroll has no effect on your PAMSA membership or employment.
- By enrolling, you agree to these terms and to the practices described in our Privacy Policy.
Eligibility
- The benefit is available to eligible PAMSA members and covered adult dependents enrolled in the qualifying health plan.
- Mino MD verifies eligibility before care begins. If coverage changes or ends, access to the benefit may end as well.
- A Mino MD physician confirms clinical appropriateness before starting any program; not every member qualifies for every program.
Cost
- Covered services are billed through your existing health plan, so your out-of-pocket cost is $0 — no copay and no deductible.
- Services that fall outside the covered benefit will always be explained to you before they are provided.
- If your plan denies a claim for reasons outside our control, the Mino MD care team will contact you before any charge is applied.
Appointments and cancellations
- Please cancel or reschedule at least 24 hours before your visit so the time can be offered to another member.
- Repeated missed appointments may pause your enrollment until you re-confirm with the care team.
- You may withdraw from the program at any time by contacting the Mino MD care team.
Your responsibilities
- Provide accurate, complete information about your health, medications, and contact details, and keep them up to date.
- Follow the care plan agreed with your physician, and tell your care team about side effects or changes in your health.
- Do not share prescribed medications with anyone else.
